GDNA launches a voluntary Fraudulent Prescription Documentation Form

The Georgia Drugs & Narcotics Agency has created a form for pharmacy staff to document fraudulent prescriptions. This form may be completed and kept on file within the pharmacy or sent to the appropriate police department, sheriff's office, or other law enforcement entity. This form is voluntary.
